Dear Sport Club Officers,
Thank you for all of your
hard work this season and continued dedication to the success of your
club.
In an effort to help make
the end of the year report less of a daunting task for you, we have
generated a template for you to use. All you need to do is carefully
follow the directions within the template and fill in your teamâs
information. Please feel free to add photos of your team; we encourage
you to use at least a team photo and pictures taken during community
service.
Please spiral bind the
report or use a report cover with clear plastic front, and turn
in 2 printed versions and 1 digital (on a CD), to me by Friday, May
29, 2009. Thanks again for all of your effort and time.
Best Wishes,
Jake Mailhot

Greatest
Accomplishment of the Season
Detail what
you feel was the clubâs greatest achievement for the season, for example:
We successfully
hosted our first home meet. Seven schools were in attendance, with X
number of competitors.
WWU performed
well with 6 team members placing advancing to the next round.
So and so and so and so qualified for regionals, which took place in
November. There they took 8th and 4th, allowing
them to move onto nationals where they placed 7th and 21st.
Overall the meet was a great success rising X dollars and expanding
our network. We expect to host the same competition next year and turn
it into an annual event.
Or
Although we
lost to WSU, we feel that it was our best game due to the performance
of the team. We were able to communicate better on the field than
in any other game this season and made drastic improvement to our ball
handling and passing skills. For our team it was a great encouragement
and a well fought game.
Central Goal
Fill in your
teamâs top goal for the season, for example:
This year we
set a goal of going 8-3 this year; we attained this goal, by retaining
more participants throughout the year and by focusing on fundamental
skills.
Or
Our top goal
this season was in increase our fundraising efforts by 25%. We achieved
this by participating in the same events as we did last year, and by
creating an alumni list to send newsletters and sponsorship/donation
letters to. With the additional contact list, we were able to generate
an extra $800 dollars this season.

The Year in Review
Write a summary of the yearâs
events. Give the highlights. Were tryouts/recruiting successful? Was
retention consistent or an improvement over last year? List the number
of tournaments/competitions you competed in. Was the year financially
successful? Were there drawbacks, obstacles, and other difficulties?
How did you overcome them or plan to overcome them? Did you meet your
goals? Do you have a long term plan for the team? (Please keep the summary
to one page)
Community Service
Give highlights,
describe what events you put on or helped with and why you chose them.
Also include a letter of confirmation from the event supervisor if possible.
REMINDER: Each club is required to participate in at least 10 hours
of community service as a club each year with 80% of the members on
the active roster participating. Community service must be authorized
before the event by the Sport Club.
(Adding pictures from the events
is encouraged; please keep summary to one page)
For example:
Our team gives back to the
community, because we care about the community in which we practice
and compete. This year we hosted a kidâs camp to increase awareness
of the sport and to generate future talented and experienced students
who grew up playing the sport, who are excited to play, and are dedicated.
The camp was a great success, with 35 kids in attendance. We were able
to practice hand drills, skills exercises and play scrimmage games.
Fundraising
